Station 1: Participant enters MDC via door closest to passenger seat. Consent is obtained at station 1.

  • Client information is collected via the MDC iPads using the JotForm App
  • If iPads are unavailable please collect information via paperwork.
  • Start a MDC "Know your numbers" card for the patients screening results to take with them.

Station 2: Participant receives blood pressure reading. Then collect patient weight, height and BMI.

  • BMI will calculate automatically on the iPad.

Station 3: Participant receives any lab testing.

Station 4: Participant receives foot care, if applicable. Foot Assessment Procedure

Station 5: Lead nurse reviews consent form, provides education and answers any questions.

  • Make sure participant has "know your numbers card" and a copy of flu vaccine (if they received it).

Station 6: Participant exits through rear door.

Cleaning

  • Wipe down all machines.
  • Ensure all supplies/equipment are put away in CORRECT area. 
  • Bag all biohazard in large red bags.
  • Bag all trash in clear trash bags.
  • Wipe down counters using disinfectant wipes.
  • Use swiffer dry to sweep floors.
  • Use swiffer wet after sweeping to clean floors.

Safety

  • Ensure all cabinets, bins and drawers are closed securely.
  • Ensure spa chair is properly secured.
  • Ensure all supplies are properly stowed or disposed of per policy.

Operations

  • Return any supplies or equipment not stored on the unit to the MDC closet.
  • Place the PINK consent forms in the basket on the filing cabinet in the NMC closet.
  • Pleace the biohazard bag in the BIO bin in the MDC closet.
  • Discard of trash. 
  • Return dirty linen to nursing lab (if applicable)
  • Return refrigerated items to the fridge (cassettes, flu shots, etc.) (if applicable)
  • All supplies and paperwork must be returned the same day as screening unless approved by MDC Coordinator.
  • Any inventory issues should be reported to the MDC Coordinator as soon as possible for re-order/re-stock.

iPad technology is available for all screenings! Please use the App on the Ipad JotForm (click the link for instructions). Please only use the paper forms if iPads are not available.

Please review the following attached paperwork before beginning any MDC event.  This paperwork will be included in your tote. Please do not print as these are just for review. This ensures that all paperwork is completed accurately before the end of each MDC event.  Faculty is responsible for the accuracy of this paperwork and all items are to be completed/returned to the MDC Closet or MDC Coordinator immediately after event closing.

These forms are carbon copy. The white copy is for the client and the pink copy is for our records. The white copy has patient information on it and should be treated with care while in our possession.  Since clients will be given back the white copy of the form, no faculty/staff or student should be in possession of personal health information at any time post event. 

Please have the client fill out the consent form and personal information through family history. The additional questions will be asked by the faculty/staff and students throughout the screening process. The lead faculty will close out the encounter with the client and ensure all of the form is filled out. Please give the white copy to the client and show them the "understanding your results" on the back side.
