Description
In this course, students examine the development of the care coordinator role in meeting healthcare needs of the
individual and community. Students will explore the influence of the social determinants of health, health disparities
and health literacy in population health management. Legal and ethical considerations regarding confidentiality and
client privacy, client rights are investigated. Students will integrate collaboration and communication in the care
coordination process to enhance understanding of healthcare considerations.
