Currently all program evaluations are done using Survey Monkey. You will receive an e-mail at the end of your program with a link to the page containing the evaluation and any available handouts.

 

Open access to the evaluation ends after 3 business days.  If you have not completed the evaluation, you will not receive credit for the course unless you contact us within 30 days of the course date to arrange for access. Please contact us immediately if you have computer issues or are unable to complete the survey online.
